Licensed Social Worker
SUMMARY/OBJECTIVE:
Provides professional team-based care to patients and assists in the continuum of care and assists in educating patients on their patient centered medical home.
ESSENTIAL JOB DUTIES: (This list may not include all of the duties assigned.)
- Provide mental health services such as therapy, counseling and intervention services to patients.
- Provides good interpersonal skill as well as the passion and inner drive to help others.
- Assists with billing out for services performed and completing appropriate documentation in the patient’s medical record.
- Provide assistance, access to different resources and guidance.
- Attends required meetings and participates in committees as requested.
- Participates in professional development activities.
- Performs related work as required.

EDUCATION AND EXPERIENCE:
Required:
- Master’s Degree in Social Work
- Two years of post-degree clinical experience
Preferred:
- Word processing and computer experience
CERTIFICATE/LICENSE:
- Mandated State licensing and certification required
WORK ENVIRONMENT:
Exposure to communicable diseases, toxic substances, and other conditions common to a clinic environment.
