Adams County Integrated Health Care Services is recruiting for a Care Coordinator for their WISe Program. In this position a qualified candidate facilitates and provides advance care coordination activities for individual youth and families. The WISe Coordinator is part of a team that provides intensive wrap around services and support to youth and their family by listening, encouraging, coaching, empowering, skill building, and coordinating with available resources suited to their recovery needs as identified on the Children and Adolescent Needs and Strengths (CANS) assessment. The qualified candidate will ensure the active participation in the implementation of Cross Systems Care plan while assuring quality mental health services which contribute to the wellbeing of the citizens in Adams County. Employees in this position function as a WISe Coordinator and provide on-call response services and will require considerable contact with local and state social service provider agencies and the general public. This position is required to work various hours day and night & days of the week. The schedule for this position will be set by the WISe Supervisor or designated manager and may be changed as Agency/department needs change. Bachelor degree in education, psychology, social work or related field plus three (3) years of community/educational outreach experience or, a combination of education and experience that demonstrates the required knowledge, skills and abilities required. Two years of direct services experience with children and families of targeted population, knowledge of family support principles and practice, early childhood development and family development, Have a high level of flexibility and accessibility, to include ability to work evenings and weekends to meet the needs of youth and families where and when they need support. Must be able to handle stressful and crisis situations tactfully and appropriately. Have a strong understanding of Wraparound Intensive Services. Have excellent interpersonal and communication skills with ability to work with individuals on a one-on-one basis or collaboratively as part of a team. Must possess a valid driver's license. Must successfully pass a background check and a driving record check. Bilingual Spanish encouraged to apply.
Beginning Salary range: $38,064.24 43,472.16 depending on experience.
